Care Pathways

Pregnancy and Childbirth Care Pathway

Overview of the care pathway

We can divide the pregnancy and childbirth care pathway into 5 phases:

  1. First prenatal visit
  2. Early 3rd trimester
  3. Prior to discharge from birth unit
  4. 1st postnatal checkup
  5. 6 months follow-up

The backbone of the care pathway is summarized in the figure below, for this flow we relied mainly on the ICHOM standard set of Patient-Centered Outcome Measures for pregnancy and childbirth.

From your EMR to scheduling or billing, Awell easily connects with your systems so you can create powerful clinical guidelines that tie disjointed systems together.

The following patient-reported-outcomes are visualized in the care pathway:

  • PROMIS-10
  • International Consultation onIncontinence Questionnaire -Urinary Incontinence Short Form(ICIQ-UI SF)
  • Wexner
  • Breastfeeding Self-Efficacy Scale -Short Form (BSES-SF)
  • Mother-to-Infant Bonding Scale(MIBS)
  • Patient Health Questionnaire-2(PHQ-2)
